| Description: | As a member of the Living Center team, this person is responsible for providing direct and comprehensive social work services to adult and geriatric populations within a nursing home setting. Job responsibilities include the following: completes pre-admission screening; performs resident assessments and develops a treatment plan; provides direct care and counseling to residents and families; leads the interdisciplinary resident care conferences; and provides education to residents, family and staff regarding psychosocial issues and needs of the residents. This position also coordinates and facilitates communication among medical staff, members of the interdisciplinary team, and the residents/families. Evaluates residents quarterly for appropriate discharge to the less restrictive environment and facilitates discharge planning to meet resident needs. Participates in performance improvement and adheres and practices according to SJMC Corporate Compliance Program. |
| Qualifications: | Clinical Social Worker with a Masters degree, licensed in Wyoming with minimum of 3 years experience in a skilled nursing facility, or alternative health care setting for Geriatric Social Work. |
